2013 Forms for CME Credits
The following forms must be filled out by those requesting CME credits for sessions at the 2013 Annual Meeting:
1. The brochure lists the sessions qualifying for CME credits and contains the page with payment information. This must be filled in and payment made.
2. The Overall Evaluation Form must be completed and the attendee should indicate the total number of credits requested.
3. The Speaker Evaluation form must be completed for those sessions that the attendee has taken for credit.
